6. Appointment Booking and Cancellation Policy

6.1 Booking Procedures
To book a hair transplant procedure, you must:

Complete a consultation (online or in-person)
Receive approval from our physician
Pay a non-refundable deposit of $600
Sign appropriate consent forms

6.2 Deposit Payment Methods
Deposits can be paid via:

Zelle
Cash
Cashier's check
Money order
Credit card (authorized processors only)

6.3 Cancellation and Refund Policy

FULL REFUND (100% of deposit):

Cancellation or rescheduling made 14 days or more before scheduled procedure date
Cancellation on the day of surgery due to medical reasons that prevent you from safely undergoing the procedure (as determined by our physician)

PARTIAL FORFEIT ($500 deposit forfeited, $100 refunded):

Cancellation or rescheduling made less than 14 days before scheduled procedure date

COMPLETE FORFEIT (100% of deposit forfeited):

Failure to appear for scheduled procedure without prior notice
No-show without communication

6.4 Rescheduling
You may reschedule your appointment following the same policy as cancellations. We recommend providing as much notice as possible to secure your preferred new date.

6.5 Facility Cancellation
If NHT must cancel or reschedule your procedure due to circumstances within our control (e.g., physician unavailability, facility issues), you will receive:

Full refund of your deposit, OR
Option to reschedule at no additional cost

Online Consultation Process

7.1 Consultation Request
When you submit an online consultation form, you:
• Provide basic information about yourself and your hair loss
• May upload photos for evaluation
• Consent to our use of this information for consultation purposes

7.2 Video Consultation
Following your online form submission:
• We may request additional information via a video consultation checklist
• You will complete a comprehensive patient information form
• A video consultation will be scheduled with our physician
• Preliminary evaluation and recommendations will be provided

7.3 Limitations of Online Consultation
Please understand that online consultations have limitations:
• Precise measurements and hairline design require in-person evaluation
• Final graft count and coverage area are determined on the day of surgery
• Medical conditions may require in-person assessment
• We reserve the right to require an in-person consultation before proceeding

7.4 No Obligation
Submitting a consultation request does not obligate you to proceed with treatment, nor does it obligate us to accept you as a patient.

8. Payment Terms

8.1 Pricing
Current pricing information is available on our website and will be discussed during your consultation. Prices are subject to change without notice.

8.2 Payment for Services
Full payment for procedures is typically required before or on the day of surgery unless other arrangements have been made in writing.

8.3 No Insurance Coverage
Hair transplant procedures are considered cosmetic and are typically not covered by health insurance. You are responsible for all costs associated with your procedure.

8.4 Financing
We do not directly offer financing. However, we can provide information about third-party financing options that may be available to you.

9. Patient Responsibilities

9.1 Accurate Information

Providing complete and accurate medical history
Disclosing all medications and supplements you are taking
Informing us of any allergies or medical conditions
Updating us about any changes to your health status

9.2 Following Instructions

Adhering to pre-operative preparation instructions
Following post-operative care instructions
Attending all required follow-up appointments
Contacting us promptly if complications arise

9.3 Medication Management
• Discussing all current medications with our physician
• Following recommendations regarding hair loss prevention medications (e.g., finasteride, minoxidil)
• Understanding that medication compliance may be necessary for optimal long-term results

9.4 Realistic Expectations
• Understanding that results vary by individual
• Accepting that multiple procedures may be necessary
• Recognizing that natural hair loss may continue despite surgery
• Acknowledging that transplanted hair requires time to grow (typically 12-18 months for full results)

10. Informed Consent

Before any procedure, you will be required to sign an informed consent document that includes:

Description of the procedure
Potential risks and complications
Expected outcomes and recovery timeline
Alternative treatment options
Your opportunity to ask questions
Your voluntary agreement to proceed

No procedure will be performed without your informed written consent.

11. Limitation of Liability

11.1 Medical Decisions
NHT and its physicians, employees, and agents are not liable for:

Your decisions regarding whether to undergo treatment
Your compliance or non-compliance with pre- or post-operative instructions
Complications arising from undisclosed medical conditions
Results that do not meet your expectations despite proper procedure execution

11.2 Website and Information
TO THE FULLEST EXTENT PERMITTED BY LAW:
The Site and its content are provided "AS IS" without warranties of any kind

We do not warrant that the Site will be error-free, uninterrupted, or secure
We are not liable for technical problems, service interruptions, or data loss

11.3 Maximum Liability
In no event shall NHT's total liability to you for all damages, losses, and causes of action exceed the amount you paid to NHT for services, or $600 (the deposit amount), whichever is greater.

Loss of profits, revenue, data, or business opportunities
Emotional distress or personal injury (except as required by law)
Damages arising from third-party actions

11.5 State-Specific Limitations

12. Indemnification

You agree to indemnify, defend, and hold harmless NHT and its officers, directors, employees, agents, and contractors from any claims, liabilities, damages, losses, costs, or expenses (including reasonable attorneys' fees) arising from:

15.3 Informal Resolution
Before filing any formal legal action, you agree to first contact us to attempt to resolve any dispute informally. Many disputes can be resolved this way more quickly and cost-effectively.

15.4 Class Action Waiver
To the extent permitted by law, any dispute must be brought in an individual capacity and not as a class action, consolidated action, or representative action.
